How to Apply

Note that the information on this page applies only to applicants who wish to be accepted to a PhD or Master's program in Mathematics.

Materials to be submitted online:  Test scores to be submitted directly from ETS:

All Applicants

  • Complete the University of Washington Application for Admission to Graduate School online, and pay the nonrefundable $65 application fee.
  • A statement of purpose (part of the online application), consisting of about 200 to 300 words on a separate sheet, detailing your reasons for wishing to enter graduate school in mathematics, your plans for the future, and any other information about your educational and professional objectives that may be relevant to your application.

Arrange for ETS to send the following scores directly to institution code 4854:

International Applicants (department code 72)

US Applicants

You do not need to mail any materials to Graduate Admissions if you complete the application and pay the fee online by credit card, unless you have earned a bachelor's or higher degree from an institution outside the United States or its territories. If so, then you must print a copy of the application and send it, together with an official copy of the transcript and degree statement, to the address above.

International Applicants

  • A copy of your completed University of Washington Application for Admission to Graduate School. 
  • International transcripts with degree statement, including date of graduation and title of academic degree awarded.
  • Transcripts from the U.S. and Canada: ask the Registrar to mail official transcripts in a sealed envelope to this office.
  • Your completed University of Washington Statement of Financial Ability. (Once you have submitted your application online, you must log back into the online application to access the form.) Even if you are applying for financial support from the Math Department, you must complete this form to ensure an accurate student visa application.
  • Photocopies of all visa status forms issued to you.
