Exam Administration

University of Washington Department of Mathematics, Examination Administrator Agreement

Students needing to take tests for a UW Mathematics course at a site other than on the UW Seattle campus with their assigned course, must have an approved proctor administer the exam.  It is the responsibility of the student to secure an acceptable exam administrator and ensure that all agreement forms are completed and submitted to the course instructor.  The student must pay all expenses associated with administering exams.  

Exams taken at alternative locations must be taken at the same day and time as scheduled on the course syllabus. 

Exam administrators must be one of the following:

  • College, professional testing center staff or test administrator
  • Professional learning or tutoring center
  • Military: an Educational Services Officer (ESO)
  • NCAA: Coach, Advisor or other approved NCAA staff member
  • Educator: a state certified teacher or counselor at an accredited educational institution

Exam administrators must have and use the professional phone and email address which corresponds to the institution where the individual is employed and through which the individual has the job title qualifying to be an exam administrator.

Exam administrators may not be:

  • A fellow student, friend or peer
  • A relative, partner or roommate of the student
  • A co-worker or employee of the student

Students and the exam administrator are responsible for following ALL exam protocols.  Any violation of exam protocols, including but not limited to use of prohibited aids, receiving the exam in advance of the approved start time, working on the exam beyond the exam duration, having someone other than the student take the exam; will result in academic misconduct.  In addition to being reported to the University's Community Standards and Student Conduct office, the instructor and Department have the authority to determine a penalty at their discretion.

Procedure for seeking approval

  1. Student secures an acceptable exam administrator for the days and times that match the exam, then completes and submits the Student Agreement form.
  2. The exam administrator receives the Student Agreement form via email, reads the test proctoring procedures, replies-all (including the instructor and Department contact) to the email received agreeing to be the exam administrator and follow all exam protocols.
  3. Instructor receives email from exam administrator agreeing to the in-person exam.
  4. Instructor notifies the student of the decision.  If approved, Student confirms the day/time with the exam administrator.

Accommodations: If you have testing accommodations already in place with Disability Resources for Students, you are responsible for confirming the exam administrator has the equipment and/or facilities to provide your accommodation.  

After approval

  1. The instructor, or designee, will send the exam to the exam administrator with exam instructions, including contact information for day-of exam questions or problems.  If the student has testing accommodations, these instructions will be included with the exam.
  2. The exam administrator will administer the exam at their institution.  Test must be taken in a quiet and secure environment, ensuring that all testing instructions are followed.
  3. The exam administrator will maintain the integrity of the tests at all times.  Students are only allowed access to the tests at the time of administration, following all exam instructions related to access to electronic devices, note sheets, etc.
  4. If there is a problem, the exam administrator must immediately notify the instructor via email or phone.
  5. The exam administrator is responsible for returning the test back to the instructor, via scanned PDF.
